Staples High School |
| Staples High School requires several
forms to be completed
before participating on school athletic teams. The HAR - 3 Medical Evaluation form is also required but must be on blue paper. The form is available from the school nurse. The link above will also take you to the Travel Release form if the student will not be using school transportation to or from a school athletic event. A Skin Condition form is required to be completed by a physician before any wrestler is allowed to participate after diagnosis of any skin condition. |
